No, the Hematoma does NOT go away by itself. It has to surgically opened and drained by a vet. Then there will be antibiotics and a drain tube in your puppy's ear until completely healed.

Get a vet appointment now.

im not sure what you should do other than go to the vet. I have a shar pei and this forum has been really helpful for any issues or questions you have about shar peis.

Sometimes the vet can remove enough fluid from the hematoma with a syringe that surgery is unnecessary.

Yes, it hurts.

Failure to deal with it can cause it to get bigger and can cause significant scarring.

You need to take him to the vet. They will either do surgery or lance it open and drain it. Make an appointment and ask your vet what all the surgery entails. May not even require an overnight stay at the hospital. Needs to be taken care of right away or can get worse.
